St George Community is a registered NDIS provider delivering flexible, person-centred supports across the St George area.

We work alongside participants, families and carers to provide practical support that builds confidence, independence and meaningful community connections.

Who it’s for

Our NDIS supports are for people with an NDIS plan who would like one-on-one assistance at home, in the community, or both. We work alongside participants, families and carers to provide support that reflects each person’s needs, preferences and circumstances.

How we support you

Supports can be delivered at home, in the community, or a combination of both depending on what works best for each participant. Services may be short-term or ongoing and are designed to support independence, wellbeing and social participation.

Our services include

  • Support to access the community using our transport services
  • One-on-one assistance to attend appointments, social outings and activities
  • Help at home with cleaning, cooking and meal preparation
  • Support with shopping, errands and everyday tasks outside the home
  • Personal care at home, including showering, grooming and daily routines, delivered with dignity and respect

Tim’s story

When Tim moved into his group home at the age of 52, he had spent most of his life living in an institutional setting and had limited opportunities for community participation.

Through the support of The Disability Trust and our NDIS one-on-one services, Tim began to build routine, confidence and meaningful engagement in his day-to-day life.

For more than five years, Tim has participated in regular weekday outings with his long-term support worker, Patricia. These outings have helped Tim access the community, build social connections and take part in activities that support his physical health, emotional wellbeing and independence.

Today, Tim leads a fuller and more active life. He is more socially connected, more engaged in his home environment and shows greater confidence and wellbeing.

Anita’s story

When Anita* first connected with our NDIS team, she had not left her house for six months.

The world beyond her front door felt overwhelming and out of reach. With patient, personalised one-on-one support, that first step outside became the beginning of real change. Confidence grew, trust was built and community slowly became part of everyday life again.

Today, Anita attends social outings several times a week and has built a strong, supportive relationship with her support worker, someone who understands her, moves at her pace and supports her goals.

Anita also uses our one-on-one support to attend weekly therapies, including occupational therapy. Since connecting with St George Community, she has been encouraged and supported to build greater independence at home, including accessing practical tools such as a reading aid to support daily life.
